This is sinus rhythm, second degree AV block 2:1. This is often read as Mobitz II. However, 2:1 AV block is a subtype of second degree AV block. A 2:1 AV block can either be due to a Mobitz I or Mobitz II mechanism. A longer strip is needed to capture the mechanism.
